The Social Services Assistant supports the Social Services Department in meeting the psychosocial needs of residents in a skilled nursing facility. Responsibilities include assisting with resident and family communication, psychosocial assessments, care planning, discharge planning, documentation, referrals, and coordination of services. The position works closely with residents, families, nursing staff, interdisciplinary team members, and outside providers to help ensure appropriate support and continuity of care.
Additional responsibilities include responding to resident and family concerns, assisting with appointments and community resources, maintaining accurate and timely documentation, participating in care plan meetings, and supporting the Social Services Director with daily departmental operations.
English & Cantonese Required | Mandarin Preferred
